Registration

Patients wishing to register with the pratice should ask at reception for the appropriate forms. They can also be downloaded from the link to the right. Please remember to use one for for each member of the family you wish to register.

Patients now register with the practice and not a specific doctor. You may express a preference to see a particular doctor when you make an appointment.

Please be aware that the regulations around registering have recently changed; not every one who lives in the UK is eligible for free NHS care. We are now asked to see photo ID for all new adult registrations, children do not require ID.

Suitable ID for us would include:

  • Passport
  • Driving licence
  • Any photo ID showing name and date of birth
  • Any Government department letter
  • Utility Bill
  • Bank or credit card statement

Similarly if you are foreign national you may not be eligible for free NHS care. The receptionists can help you with this if you require any more information.

Change of Address

Please let us know as soon as possible if you change address pr telephone number. If you move away out of the practice area you should register with a doctor nearer your new home. You will automatically be removed from our list after 3 months.

NHS Certificates (sick lines)

 

You do not need a Doctors Certificate unless you have been absent from work for 7 days. If your employer insists on a certificate THEY may contact us (with your permission) and we can suply a private certificate for which they will have to pay.

Please to NOT make an appointment simply for a Doctors certificate unless you haev been off work for more than 7 days.

You no longer require to be signed back to work after an absence.

Specimens and Test results

If you are asked to hand in a specimen, please do so before 11.30am or your results may be delayed. Your doctor will probably have told you how to get the results, if not please contact reception allowing for the sample to be processed and the results returned to us. If doing so by phone please call after 2.30pm.

Please read our item on confidentiality if you would like some one else to get your results on your behalf.

Travel Immunisations

The practice nurses can offer vaccinations for most of the routine travel requirements. A travel form can be obtained from reception for completion or downloaded here. Please complete this and contact the nurse at least 8 weeks before you travel. Note there are 2 forms to download for this.

Please be aware a charge will be made for those vaccinations not covered by the NHS and ALL antimalarial prescriptions.

Fees and Professional Services

Services provided that are not covered by the NHS will be charged at the appropriate rate. Non-NHS services include HGV medicals, PSV medicals, motor racing medicals, Insurance examinations and reports, private certificates, vaccination certificates etc.

A list of the fees is available at the reception desk. Please note all of the above services must be pre booked in advance and the receptionist will make you aware of any fee bookable at the time of booking.

We are always happy to hear your comments and suggestions on any aspect of the Health Centre. A suggestion box is situated to the left of the reception desk or you may contact the practice manager who will be pleased to discuss this with you.
